For your first appointment:

  • Please arrive 10 minutes early to fill out our intake forms.
  • Allow 60 to 90 minutes for your first appointment.
  • Gowns and shorts will be provided.

Please bring these items with you:

  • Photo ID (per federal regulations)
  • Prescription from your physician
  • Insurance cards
  • List of medications and pertinent medical history
