For your first appointment:
- Please arrive 10 minutes early to fill out our intake forms.
- Allow 60 to 90 minutes for your first appointment.
- Gowns and shorts will be provided.
Please bring these items with you:
- Photo ID (per federal regulations)
- Prescription from your physician
- Insurance cards
- List of medications and pertinent medical history
Download, complete, print, and bring the following forms needed for your first visit:
